[1] Take a new file of 500 pixels, 500 pixels, of resolution 72 dpi in the RGB mode.

[2] Now open your image like this.

[3] Take the Elliptical Marquee Tool create a shape.

[5] Then delete a background image.

[7] Go to Distort>Spherize use these settings.

[24] Go to Blending Option use the following settings.

[25] Your image should be look like this, now select the Background Layer and fill with Black color.

[26] Now go to Filter>Noise>Add Noise use these settings.
