[1] Take a new file of 500 pixels,300 pixels, of resolution 72 dpi in the RGB mode.

[2] Fill the Backgroung with black color, now take the Rounded Rectangle Tool create a new shape as shown image below.

[3] Now go to Blending Option apply the following settings.

[18] Now make the three copy of small circle layer and move it the different distance as shown image below.

[20] Go to Blending Option apply the following settings.

[21] Now make the two more copy of small circle layer and arrange the cirlce as shown image below.

[22] Your image should be look like this.

[23] Now take the text tool and write your text your final image is ready.
